Forms and Instructions for the Survey of Local Government Finances
The UT-1, UT-2, UT-4, UT-5, and UT-6 forms can now be completed electronically (using Microsoft® Excel®) and e-mailed to the Utah State Auditor's Office. E-mail the completed file to utforms@utah.gov.
